What Are Medical Notes?

Medical Notes are detailed records of all your healthcare interactions on MedxVerse. They include consultation summaries, doctors observations, diagnoses, treatment plans, prescriptions, and follow-up instructions. These notes form a complete picture of your health over time.

Types of Medical Notes

Consultation Notes

After each consultation, your doctor creates detailed notes documenting your presenting complaints and symptoms, medical history discussed, examination findings (when applicable), diagnosis or differential diagnoses, treatment recommendations, prescribed medications, lifestyle advice, and follow-up instructions.

Prescription Records

Every prescription issued through MedxVerse is saved with complete details including medication name and strength, dosage instructions, quantity prescribed, prescribing doctor information, date issued, and refill authorizations.

Lab Results

Lab tests ordered through MedxVerse are automatically linked to your records including the test type, date, results with reference ranges, doctor’s interpretation, and trend tracking for repeat tests.

Health Diary Entries

You can add personal health observations such as daily symptom tracking, vital signs (blood pressure, blood sugar, weight), medication adherence, lifestyle notes (diet, exercise, sleep), and questions for your next consultation.

Accessing Your Medical Notes

• Open Medical Records: In the MedxVerse app, tap on Medical Records or My Health.
• Browse Your Notes: View all healthcare interactions organized by date and type.
• Search and Filter: Find consultations, medications, or conditions easily.
• View Details: Tap any entry to see complete doctor notes and recommendations.
• Download or Share: Export notes as PDF or share directly with other healthcare providers.
